Eligibility for most Community Action programs and services are based on household income for the month prior to the appointment date and household size. Some programs are prioritized where there are elderly and/or children in the household. *All programs and services are based on availability of funds.*

PROOF OF ELIGIBILITY

Applicant must provide some or all of the following documents/verifications, depending on the program or service:

• Photo ID (head-of-household or spouse)
• Proof of income for all household members
• Social Security Cards for all household members
• Recent Energy Bill
• Food Stamp Allotment Letter
• Child Custody / Proof of Child Support
• Certificate of Occupancy (Renters)
• Declaration of Income (if no prior month income)
• Current Lease & Form W-9 from Landlord
• Notices: Past Due, Eviction or Vacate
